Retatrutide Dosage Came From Studies | Doctor Retatrutide

Retatrutide amounts aren’t instructions for you

What amounts did the studies use? Lilly-sponsored Phase 1b, Phase 2, and Phase 3 studies provide the retatrutide dosage figures here; those names mean small early safety work, a middle check of results and common problems, and a larger late check of both. Retatrutide isn’t FDA approved. Study doctors watched people closely while amounts rose in set steps. This page can’t pick an amount for you. I’d ask the doctor who knows your records.

Phase 2 used 2 mg as a step toward the 12 mg target

The Phase 2 weight study followed 338 people in 2023 [1]. People headed for higher targets began at 2 mg and moved up in small steps. The targets were 1 mg, 4 mg, 8 mg, or 12 mg. The lowest target was below the starting step, so that group stayed at its target. The aim was to limit nausea and other gut trouble. Each person got a shot under the skin.

At 48 weeks, higher amounts came with more average loss:

  • 1 mg: 8.7% average loss
  • 4 mg: 17.1% average loss
  • 8 mg: 22.8% average loss
  • 12 mg: 24.2% average loss
  • Placebo: 2.1% average loss

The other Phase 2 study followed 281 people with type 2 diabetes in 2023. It tested 1 mg through 12 mg each week for 36 weeks, with the amount rising in steps [3]. In the 8 mg group, 82% reached 6.5% or less. That blood test uses a percent to show sugar stuck to red blood cells over recent months. Your result could differ.

Nausea, vomiting, and diarrhea were the main limits at higher amounts. Raising the amount little by little was meant to ease those problems. The study team watched each person. You can’t use the list to choose an amount for yourself.

Phase 2 used 2 mg as a step toward the 12 mg target

Higher retatrutide amounts brought more weight loss

In Phase 2, weight loss rose as the retatrutide amount rose. A review combined three studies that placed people into groups by chance [6][7]. The broad finding held across the groups. You still won’t know your result before your body responds.

Compared with placebo, the 2025 review found 10.66 kilograms more average weight loss. Waists fell 6.61 centimeters more [6]. More people taking retatrutide reached the 5%, 10%, 15%, or 20% marks [7]. Each mark was checked on its own. Those group results can’t tell which mark you might reach.

One group with type 2 diabetes had X-ray scans of body fat, muscle, and tissue without fat. At 8 mg, body fat fell 26.1%, compared with 4.5% for placebo, at 36 weeks [8]. Muscle and other tissue made up about the same share of loss seen with approved weight drugs. The scan didn’t find an extra share from those tissues. Your scan won’t have to look the same.

Retatrutide remains in blood long enough for weekly shots

Retatrutide has a 6-day half-life. That means half the drug has left your blood by then. An early human study called Phase 1b confirmed that span in 2022 [2]. After about 4 weeks of weekly shots, the amount left before each shot becomes steady. The drug may start acting before then.

The highest blood level comes 12–72 hours after a shot [15]. Retatrutide holds onto albumin, a protein in blood. That bond slows the drug’s breakdown and makes weekly shots possible. You won’t necessarily clear it at the same speed.

Many common drugs are handled by a liver system. Lab and Phase 1 work found no sign that retatrutide blocked that system [15]. Peptide drugs such as retatrutide usually break into small protein parts, so the body handles them another way. In Phase 1b, blood levels rose with study amounts from 3 mg to 12 mg. That finding can’t cover every medicine you take.

Retatrutide eased hunger before larger weight changes appeared

People in the Phase 2 study said hunger eased within the first 1–2 weeks. That early change fits the drug’s first two actions, which can help people feel full [10]. Hunger changed before blood levels became steady.

Most people in Phase 2 had lost 5% or more by weeks 8–12. The study amount had risen in steps by then. Blood levels were also nearing their steady point [2][10].

In the type 2 diabetes Phase 2 study, people reported less hunger within the first four weeks. By week 12, those at 4 mg and above also reported fewer times when eating felt hard to control [10]. Your timing could be different from the group’s.

Some weight may return after retatrutide ends

Researchers expect some weight to return after retatrutide ends. That view comes from animal work and human studies of GLP-1 hunger drugs [17]. It isn’t a full answer for retatrutide. Related GLP-1 drugs can’t show your long-term result.

In animal tests, weight and food intake rose after retatrutide stopped [17]. In human studies of other hunger drugs, weight and some health gains faded within 52 weeks. Fat returned faster than muscle and other tissue.

A 2026 review called the weight return a return of the illness, not failed care [17]. Obesity often lasts for years. Cost, time, and the plan after treatment are fair questions for your doctor. Phase 3 follow-up after retatrutide hasn’t been published.

Every retatrutide study used a weekly shot under the skin

Every retatrutide study used one shot a week in tissue just under the skin [1][2][3]. Published work included no pill or drug put into a vein as of mid-2026. The reports don’t support changing how the drug is given.

The Phase 2 weight study used 2 mg as the starting step for higher targets [1]. At set times, the team raised the amount until each person reached the assigned target. Related GLP-1 hunger shots also rise slowly. Raising them fast can bring more gut trouble. Your doctor would need your symptoms to judge that risk.

Blood levels steady around 4 weeks after the first shot at each study amount [15]. The study’s step-up plan lists the Phase 2 groups and their 48-week results. The reason for weekly shots appears above too. Neither section gives you personal instructions.
